The application must be accompanied by:<\/p>\n<ul>\n<li><strong>valid welding license proving basic welding skill<br \/>\n<\/strong><\/li>\n<li><strong>or a proof of the valid welding course for the welding technology and the group of materials<\/strong><\/li>\n<li><strong>a valid welder certificate according to one of the mentioned standards, if the recertification is concerned<\/strong><\/li>\n<\/ul>\n<p id=\"13067\">The certificate can only be issued to a person who signs the form <a href=\"https:\/\/c-wt.sk\/wp-content\/uploads\/2026\/02\/FO-15B-Agreement-and-conditions-for-the-use-of-the-certificate-welder_editable_2025.pdf\">AJ_FO 15B Agreement and conditions for the use of the certificate<\/a>.<\/p>\n<p>The welder&#8217;s certificate is valid for 2 years and can be extended before this period expires.<\/p>\n<p><strong>Plastics welding personnel according to standard STN EN 13067: 2020<br \/>\n<\/strong><\/p>\n<p>The applicant applies for certification by completing form FO 12. The application must be accompanied by one of the following documents:<\/p>\n<ul>\n<li><strong>education: full secondary vocational school &#8211; plastic processor<\/strong><\/li>\n<li><strong>employer&#8217;s confirmation of practice as a plastics welder (at least 2 years)<\/strong><\/li>\n<li id=\"13585\"><strong>confirmation of the completion of the theoretical and practical training course in the field of plastic welding in range of at least 40 hours<br \/>\n<\/strong><\/li>\n<\/ul>\n<p>The certificate can only be issued to a person who signs the form FO 15.<\/p>\n<p>The welder&#8217;s certificate is valid for 2 years and can be extended before this period expires.<\/p>\n<p><strong>Qualification testing of brazers and brazing operators according to standard STN EN ISO 13585: 2012<br \/>\n<\/strong><\/p>\n<p>The applicant applies for certification by completing form FO 12. The application must be accompanied by one of the following documents:<\/p>\n<ul>\n<li><strong>employer&#8217;s confirmation of brazing practice<\/strong><\/li>\n<li><strong>specified brazing process (BPS)<br \/>\n<\/strong><\/li>\n<li id=\"14732\"><strong>confirmation of completion of the prescribed training in range of at least 40 hours<br \/>\n<\/strong><\/li>\n<\/ul>\n<p>The certificate can only be issued to a person who signs the form FO 15.<\/p>\n<p>The certificate of brazer has a validity of 3 years and can be extended before this time.<\/p>\n<p><strong>Welding operators and weld setters for mechanized and automatic welding STN EN ISO 14732: 2014<br \/>\n<\/strong><\/p>\n<p>The welding operator is the person who manages or adjusts any welding parameters for mechanized or automated welding.<\/p>\n<p>Weld setter is the person who arranges welding equipment for mechanized or automated welding.<\/p>\n<p>The applicant applies for certification by completing form FO 12.
